Module 4b
Fill in your forms and statements
Type your details once. The app writes the wording for every main Court of Protection form and statement, so you can copy it straight onto the official form. Everything stays on this device.
Use P's initials only, never their full name, and keep the language factual. This is drafting help from families who have been through it — it is not legal advice, and the official forms must still be signed and filed with the court.
How form filling works, at a glance
Four steps. You can stop at any point — your answers stay saved on this device.
1. Gather
Get P's initials, date of birth, address, diagnoses and who else must be told. Have any care plan or assessment nearby.
2. Type it once
Fill the boxes on this page. The app writes the wording for every form, so you never retype the same answer.
3. Copy or print
Open the official GOV.UK form, then copy each drafted paragraph across. Print a copy for your own file.
4. Send and notify
Post or upload to the court, pay or apply for fee help, then notify P and anyone on your COP14/COP15 list.
Your details
About you
e.g. mother, father, sister, main carer
About P
Never the full name — this protects P under GDPR.
Diagnosis, plus whether P can understand, retain, weigh or communicate the decision.
The application
One dated fact per line. Facts, not feelings.
At least three: name — relationship — address, one per line.
Money (for COP1A and fee help)
Capacity assessor (COP3)
Application form — wording for the free-text boxes
Copy this wording onto the official COP1 form. Anything in square brackets still needs your answer.
COP1 — APPLICATION FORM (draft wording)
Applicant: [your full name]
Address: [your address]
Phone: [phone] Email: [email]
Relationship to the person this is about: [e.g. mother, father, sister]
The person this application is about ("P"): [P's initials]
Date of birth: [P's date of birth]
Where P lives: [P's address or care setting]
What I am asking the court to decide:
[State plainly what order you want, e.g. to be appointed deputy for property and financial affairs, or a decision about where P lives and who P has contact with]
Why P cannot make these decisions themselves:
[Name the impairment or disturbance of the mind or brain and how it affects understanding, retaining, using or weighing information, or communicating a decision]
Why the court needs to decide this now:
[Set out dates and events. Say what has been tried, what has gone wrong, and what harm continues without an order]
Why this order is in P's best interests, and why it is the least restrictive option:
I have applied section 4 of the Mental Capacity Act 2005. [P's initials]'s wishes, feelings, beliefs and values are set out below. I have considered less restrictive alternatives and explain why they do not meet the need.
[P's wishes and feelings, how P expresses them, with dated examples]
