Module 10b
Care and housing referral form
One referral you can send to every provider on your shortlist. Fill it in once, send it out, and compare the answers side by side. Nothing leaves this device until you send it yourself.
Use P's initials in the first referral, not their full name. Only share fuller details once a provider confirms interest. If a social worker or commissioner is funding the placement, copy them in — providers usually need a funding contact before they will assess.
