Medical paperwork to gather

Medical paperwork is usually what the court reads first. Add each item below to your timeline as you get it, using the medical categories in the form underneath.

  • GP summary printout (ask reception for a "full summary care record")
  • Hospital and consultant letters, most recent first
  • Diagnosis letters — autism, learning disability, mental health, physical health
  • Current medication list, including doses and who reviews them
  • Capacity assessment (COP3 Part B) or any other capacity opinion
  • Therapy reports — OT, speech and language, physiotherapy, psychology
  • Discharge summaries and A&E attendance records
  • Community nurse, epilepsy, dietician or continence notes
  • Hospital passport, communication passport or health action plan
  • Any refusal or delay in treatment, with the date and who said it

Money paperwork — benefits, income and outgoings — has its own page: Benefits, income & outgoings.

Nothing here yet. Start with the documents you already have — assessments, care plans and any letter with a date on it.
