Module 10c

Training referral forms

Request autism, neurodiversity, mental capacity, Court of Protection or sensory training. One form for families and unpaid carers, one for professionals and organisations. Everything stays on this device until you send it.

Use P's initials only — a training enquiry never needs their full name or diagnosis records. Ask every provider whether people with lived experience co-deliver the training; for learning disability and autism training in health and care, that is expected under the Oliver McGowan standard.

Family training request

Never the full name.

Online live, in person, blended, half day, evenings.

Give a window rather than one date — providers can then offer you their nearest match.

Time to arrange cover, respite or transport.

Term dates, hearings, hospital appointments.

Captions, easy read, breaks, quiet space, BSL, Cymraeg.

One or more, separated by commas. The booking request goes to all of them at once.

Carer's grant, direct payments, personal budget, self-funded, asking for a free place.

Your training referral

Anything in square brackets still needs an answer. Send the same referral to each training provider so you can compare quotes fairly.
